I lost my activation code, or it expired. What should I do?
If you lose your activation code or it expires, select “Create an account” from the login screen. Follow the
instructions to verify your membership and finish setting up your online account.
My activation code does not work. What should I do?
As a security measure, your activation code expires after 45 days and is no longer valid after the first time you use
it. Select “Create an account” from the login screen, then follow the instructions to finish setting up your
account.
What do I do if I get locked out of my online account?
To unlock your online account, please contact Customer Care at the phone number listed on the back of your member ID
card.

How do I delete my online account?
We strongly encourage all members to keep your online account, even if you no longer have benefits through Sharp
Health Plan. If you decide to proceed with deactivating your online account, you can do so by logging in and going
to Security settings from your account menu. At the bottom of the page, you will see a link for account
deactivation.
