How do I join Active&Fit Direct?

To join, you need to use a special Active&Fit Direct link from your Sharp Health Plan online account.

How do I find an alternative care provider?

To find a provider, you need to create a ChooseHealthy account with a special link from your Sharp Health Plan online account.

Log in from a desktop or mobile browser (not the app). Scroll down the home screen and select Prevention & wellness. In the Discounts section, select Wellness deals. A new window will open a special link to the ChooseHealthy program website.

For help finding a provider, call ChooseHealthy at 1‑877‑335‑2746, Monday to Friday, 5 am – 6 pm Pacific time.
