Here’s how your network, plan medical group and primary care physician work together.

Primary care physicians are affiliated with both a plan medical group and a plan network.

Find out which doctors are affiliated with a certain plan medical group (PMG) and plan network by using our online provider network directory.

You can change your primary care physician at any time by logging in to your Sharp Health Plan online account.

If you choose a primary care physician in a different plan medical group — for example, if you're switching from a doctor in Sharp Rees-Stealy Medical Group to a doctor in Sharp Community Medical Group — your current referrals or prior authorizations for specialists, special supplies or equipment will be voided. You must ask your new primary care physician for new referrals or prior authorizations.

Your care is coordinated through a physician group called a plan medical group, or PMG.

You receive primary care, specialty physician care and access to hospitals and other facilities through several physician groups called plan medical groups (or PMGs). PMGs are comprised of doctors, specialists and other providers who are located throughout San Diego and southern Riverside counties.

There are four plan medical groups in Sharp Health Plan's Performance Network:

  • Sharp Rees-Stealy Medical Group
  • Sharp Community Medical Group
  • SCMG Palomar Health Medical Group
  • SCMG Graybill
  • SCMG Graybill Temecula
  • SCMG Inland North Medical Group
  • Rady Children’s Health Network/Children's Physicians Medical Group (CPMG)

Plan medical groups are affiliated with plan networks.

A plan network is the group of doctors, medical groups and hospitals that are available to you as a Sharp Health Plan member. CalPERS members have access to the Performance Network.

The Performance Network covers specific geographic areas across San Diego and southern Riverside counties. You receive covered benefits and services from physicians who are affiliated with your plan medical group (PMG) and who are part of their plan network.

You choose a network when you sign up for benefits each year. Once you make your selection, you cannot make changes until the next open enrollment, unless you qualify for a special enrollment mid-year.
