Carry your member ID card

Show your ID card whenever you seek medical services, such as visiting your doctor or filling a prescription. Your ID card also has important benefit information, like your plan medical group and plan network.

Need a new ID card? Request a replacement card or print a temporary card from your Sharp Health Plan online account.

Set up your Sharp Health Plan online account

Track health care spending and so much more. To set up your online account, use the activation code we sent separately. You can also create an account using your new member ID#, listed on the front of your ID card.

For doctor’s appointments, health records and more: Members with Sharp Rees-Stealy Medical Group or Sharp Community Medical Group can log in or register for a Sharp account. Have a different plan medical group? Find out if they use a different patient portal.

Transfer your prescriptions

Use the pharmacy portal to locate a pharmacy close to home. Let your new pharmacy know that you need your prescriptions transferred from your old pharmacy. Also, let your primary care physician know of any existing special authorizations for prescriptions that may need to be re-authorized.

Get refills delivered: Rx Delivery by Mail is an easy, affordable way to get your medication, with the convenience of home delivery. Sign up with CVS Caremark.

Transfer any existing referrals and medical records

If you have existing referrals to see a specialist, ask your new primary care physician to re-authorize the referral with your new coverage. For medical records, please contact your previous doctor for assistance to transfer your health information to your new doctor.

  • Schedule a visit with your primary care physician. If you’d like to switch doctors, log in to your Sharp Health Plan online account, or email or call Customer Care at 1-833-346-4322 to make the change.
  • For after-hours and weekend medical advice, save 1-833-346-4322 to your phone contacts. Talk to a registered nurse with Sharp Nurse Connection® when it’s outside regular business hours.
