The care you need when you need it

We want to make it easy and simple for you to get the care you need. Here's some helpful information to guide you to the right care at the right time.

Your primary care physician

Your primary care physician or PCP is your main point-of-contact for all of your healthcare needs. He or she will handle your care, coordinate any treatments, refer you to a specialist, if you need one, and more. You can contact your PCP’s office 24 hours a day. If your PCP is not available, you will receive a return call within 30 minutes. You can find your doctor listed on your member ID card or by logging in to SharpConnect.

Not sure if a particular treatment or service is covered under your health plan? Your Member Handbook puts this information at your fingertips. If you would like a copy mailed to you, call Customer Care at (855) 995-5004.

Need to find a new doctor? Use our helpful doctor search tool or download a provider directory for your network.


Provider network directory

Download your Performance provider network directory that lists all primary care physicians, hospitals, urgent care centers, and pharmacies in your network.

Some hospitals and other providers do not provide one or more of the following services that may be covered under your plan contract and that you or your family member might need: family planning; contraceptive services, including emergency contraception; sterilization, including tubal ligation at the time of labor and delivery; infertility treatments; or abortion. You should obtain more information before you choose a plan. Call your prospective doctor, medical group, independent practice association, or clinic, or call Customer Care at (855) 995-5004 to ensure that you can obtain the healthcare services that you need.

Urgent care

It's best to contact your PCP first to determine if a visit to urgent care is necessary. Urgent conditions are not emergencies, but may need prompt medical attention. Urgent care centers do not require an appointment and can handle a variety of conditions. Be sure to check whether you'll need prior authorization before you visit. Find one near you.


Emergency care

A hospital emergency room provides fast, life-saving care, 24/7. If you are in life-threatening danger or at risk of being permanently disabled, then it's an emergency and you should call 911 or go to the nearest emergency room immediately. Find a hospital near you.



When you're looking for convenient treatment for a minor illness or injury, you can visit MinuteClinic®. These are walk-in medical clinics located inside select CVS/pharmacy® stores nationwide. Most services will require a $40 copay (Medicare members will pay their urgent care copay) while flu shots have a $10 copay.


Travel and out-of-area coverage

Medical issues can happen at any time. No matter where you are, your plan will cover urgent and ER care. But there are facts you need to know when you're seeking non-emergency care. Find out how to get care when you're outside of the Sharp Health Plan coverage area.


Chiropractic and acupuncture benefits

You have access to acupuncture and chiropractic benefits at a $15 copay for 20 combined visits per calendar year.
