Hold on to your Sharp Health Plan member
ID card

Your member ID card is your key to accessing care. Carry it with you at all times. You will need this card whenever you seek medical services like visiting your doctor or filling a prescription.

Get to know your plan

Your primary care physician (PCP) is your doctor and your point of contact for all of your health care needs. The Member Handbook is your Evidence of Coverage. It has all of the information you need about your benefits and coverage, and how your plan works. The Member Handbook can be found under Your benefits.

Start using your health plan

You’re ready to start using your plan. To schedule an appointment with your doctor, just call the telephone number listed on the bottom right corner of your Member ID card. Bring your member ID card, any medical records, and medications you are taking to your first visit with your doctor.

Your gateway to award-winning care


If you are in the middle of ongoing medical treatment, call your primary care physician (PCP) to let him or her know. Be sure to explain your needs, current treatments or planned surgeries. Your PCP will assess your condition and provide treatment or referral to a specialist.

Securely manage your health care online with FollowMyHealth. Available to Sharp Rees-Stealy and Sharp Community Medical Group members only, this is an easy-to-use online portal and mobile app for Apple and Android that lets you manage doctor appointments, health records and more.


Log in to your account to see plan coverage details

Maximize your membership by registering with our secure member portal SharpConnect. Here you can view your covered benefits and see your cost-sharing financial responsibilities, e.g., copays, coinsurance, deductible, maximum out of pocket, etc.


Language assistance

We provide free interpreter and translation services for all members. For assistance, call Customer Care at (800) 359-2002 and let us know your preferred language when you call. We’re glad to help.
